Event Liability Insurance
AHS is pleased to provide liability insurance coverage for official chapter events. When an event venue such as a church, community hall or college facility requires a formal Certificate of Insurance (COI), please request it here. You will need the venue name, address, and contact information, and the event name, date(s), hours, and expected attendance in order to complete this form. You may optionally upload a venue contract or other documents as needed. Certificates are typically available within a week, but please do request it no less than two weeks prior to the event; COI's may be requested months in advance.
Such a certificate is not needed for chapter meetings and other general chapter activities in a home, private studio, or a venue not explicitly requesting one.
Chapter Report Forms
These two forms are essential for AHS's service to chapters.
Annual Chapter Contacts Form (submission required annually by November 1st)
This simple Annual Chapter Contacts Form is a required submission by November 1st each year. AHS uses this information for its filing of an IRS Form 990-N tax return for every chapter, and for submitting chapter details to the IRS each year to maintain chapters' nonprofit status. This contact information is also used to keep chapter listings up to date on www.harpsociety.org. Failure to submit this form for two consecutive years may result in the chapter being deemed inactive per AHS policies.
Chapter Activities & Chapter of the Year Form (please submit annually by May 15th)
Although submission of this spring form is now optional, all chapters are encouraged to complete it. The information requested about chapter activities, challenges and needs informs AHS's future planning and budgeting. Painting a clear picture of AHS chapters is also useful for some grant applications, and for maintaining insurance coverage for chapters. Chapters may now also also use this form to apply for the Chapter of the Year award.

Chapter Leadership Seminar
The Chapter Leadership program seeks to strengthen connections between the national AHS and its local chapters. The Chapter Leadership Seminar is presented by the AHS Regional Directors and includes information on AHS programs and resources available to chapters, chapter responsibilities and mission, ideas for meetings and events, and guidance on sustaining or developing a healthy chapter.
